Get to know Home Care!

 

Children’s Mercy Home Care is the only full-service pediatric home-health agency in the region.  We utilize the services of highly trained registered nurses and therapists who assist patients and families with their medical needs as they transition to their home environment. We provide comprehensive medical care, such as infusion therapy and wound care, for a diverse group of children, some with complex medical needs. Other team members include nurse Liaisons, physical, occupational, speech and neonatal pediatric respiratory therapists, as well as experts in pharmacy, medical social work and durable medical equipment and supplies. Using a multidisciplinary approach, we actively coordinate the patient’s individualized plan of care.

 

We provide short-term medical care, education, encouragement, and support to help our patients and families attain the highest level of health and independence. The Children’s Mercy Home Care Team teaches the child’s parents or legal guardians how to independently provide the care ordered. Our goal is to prevent hospital readmissions and ensure the best possible outcomes.

 

More about the Home Health Nurse Specialist – Intake Team position

Our intake team Home Health Nurse Specialists receive and coordinate home health referrals from physicians, hospitals, facilities, and other community referral sources.  They work with hospital care managers to help coordinate hospital discharges and assure a seamless transition to home by scheduling clinical services with the caregiver, coordinating any teaching needs, and working with insurance providers. They use their knowledge, judgment, and critical thinking skills to make decisions regarding patient care.  This could include integrating relevant regulatory and accreditation standards, resources, insurance coverage, and data.  The intake team schedules home visits for clinical staff and DME (if applicable) and assigns staff to referrals.  They also assist with enteral and infusion pump teaches and troubleshooting.  Our intake team support the field staff from the office, by helping coordinate care and communication with our referral sources and caregivers.  

 

Our intake team Home Health Nurse Specialists work with an experienced and positive team. They enjoy helping families become established with a home nurse and therapy services so that they can remain out of the hospital and in the comfort of their own home. Hearing the success stories of patients who had a rough start and are now thriving because of their home care services is extremely rewarding.  They are the ‘air traffic controllers’ of Children’s Mercy Home Care!

Responsibilities

  • Develops and implements a comprehensive plan of care for the patient/family which promotes independence and optimal patient outcomes.
  • Promotes therapeutic, safe, efficient, cost-effective and family centered care.
  • Meets the educational needs of patient/family, utilizing principles of teaching/learning.
  • Promotes CMHC through service excellence.
  • Participates in performance improvement activities.
  • Attends education programs, CMHC meetings and assigned committees and promotes learning within CMHC staff, new staff orientation and visiting students.

Qualifications

  • Associate's Degree and
    • 1-2 years experience
    • Current employees hired before September 1, 2013 are grandfathered and permitted to continue in the direct care nursing role.
  • Bachelor's Degree preferred for all direct care nurses hired after September 1, 2013.
    • Direct care nurses hired after this date without a BSN, must complete their degree within 5 years of their hire date.
  • One of the following: Licensed RN - Kansas, Registered Nurse Multistate License Kansas required upon hire 
  • One of the following: Licensed RN - MO, Registered Nurse Multistate License Missouri required upon hire
  • RN Compact license for all states besides KS and MO required upon hire
    • Must obtain KS or MO compact within 90 days
  • Refer to Nursing and Advanced Practice Provide Licensure and Certification policy for required Life Support Certifications Required Upon Hire
  • Specialty Certification relevant to unit's patient population (determined by Nurse Manager/Director) Preferred Upon Hire

Starting Pay

 

 

 

Remote Work/Work from Home

This is an intermittent remote position, which means that the person hired will work with his or her manager to determine a schedule that includes both at home and on-site hours at a Children’s Mercy location. The incumbent must live in the Kansas City metro area.
